You should have a clear understanding of the reason for your office visit.

        If you have undergone any X-rays, tests, or procedures that are relevant to the reason for your office visit, you should make sure that you arrange for those results to be sent to our office prior to your visit. Our office may be able to assist you in obtaining those results if you inform us of the time and place where those test were done. In some cases, it may be necessary for you to obtain the actual X-rays, lab results, or medical records themselves for the doctor to review during your visit.

        You should print and fill out our New Patient Information Forms.  Please be sure to complete the forms in their entirety. When making a decision regarding surgery, it is important that your doctor knows all the factors that may influence the outcome of the procedure you are planning.

        You should bring a calendar or day planner with you so that you can plan for and anticipate making appointments for additional tests if needed, medical or cardiology clearances you made need, pre-admission testing, and a date for possible surgery. You must understand that scheduling certain procedures requires preparation and careful consideration to achieve the best results. You should not expect that the procedure you are planning is going to be done on the day of your first office visit, or even within the next few days, depending on the complexity of what needs to be done.

        You should bring with you all insurance information, referrals, and co-payments that are required. Our office is equipped to accept credit and debit card payments. It is your responsibility to understand your own health benefits. If any of these are missing when you arrive, your appointment may need to be rescheduled.
